In order to participate in MyHealthMirror, you must complete a personal health
questionnaire of questions about your personal and family medical history related
to heart disease, diabetes and other diseases. We will use this information to help
assess your risk for the disease.

questionnaire information will be stored and used internally by MyHealthMirror
